The Proxy Voting Form for the 2006 PFHA Membership Convention is now available on the PFHA web site at www.pfha.org. This form should be used to designate a proxy for your vote on all matters other than the election of officers. Complete the form and mail to:

The form must be received in the Association office at least 10 days prior to the meeting. Please have your Proxy to PFHA no latter than January 4, 2006. Faxes and emails are not accepted.
